CONSENT AND RELEASE FORM

A consent and release form will be sent to you in the mail. Please complete
it and return it promptly. Medical forms must be submitted prior to the week at CAMPS (your own physician's forms are acceptable).
Note: To ensure your name is printed in the C.A.M.P.S. book, application must be received by August 1.

DON'T MISS THE BUS!

Bus transportation from the Boston area to Lake Ossipee Conference Center will be offered from four locations. The buses will be filled on a first-come, first-served basis. If you strongly desire a certain location, it is recommended that you send in your C.A.M.P.S. application with full deposit as soon as possible.
Bus transportation is $42 round-trip.
One-way fares are not offered.
No refunds after August 1st.
